Through education, skill-building and support, the Health Promoter empowers UNITE HERE HEALTH members to actively participate in their care. They outreach to participants both through in person interactions and telephonically, to enroll them into and educate them about their benefits. This position also develops workplace leaders in the hospitality industry to act as healthcare “Ambassadors” to further educate their coworkers about their healthcare. The Health Promoter works collaboratively with UNITE HERE HEALTH (Fund) nurses, contracted UM/CM partners, and community healthcare practitioners to facilitate communication with participants and coordinate care to improve health outcomes. Finally, the Health Promoter also recruits participants for biometric screening events, disease management programs and group self-management programs.
The purpose of this position is to outreach to participants (union members and their participants), most often on an individual basis, targeted for group self-management programs as well as individualized care coordination. The Health Promoter works collaboratively with UNITE HERE HEALTH (Fund) nurses and health center staff, contracted UM/CM partners, and community healthcare practitioners to facilitate communication and coordinate care to improve health outcomes. The Health Promoter will work with participants through face-to-face interactions and telephonic follow up. The Health Promoter will also work with participant leaders to expand participation in and knowledge of Fund programs and benefits.

ESSENTIAL QUALIFICATIONS
Salary range for this position: Salary $63,400 - $79,300. Actual base salary may vary based upon, but not limited to: relevant experience, qualifications, expertise, certifications, licenses, education or equivalent work experience, time in role, peer and market data, prior performance, business sector, and geographic location.
Work Schedule (may vary to meet business needs): Monday~Friday, 7.5 hours per day (37.5 hours per week). The role is hybrid. Each week, the Health Promoter will work several days “in office” (a variety of physical buildings in the Boston area) to be able to work with participants through face-to-face interactions. Additionally, they will work a few days’ work-from-home, where Health Promoters will be conducting research and be on telephonic follow up calls with participants.
